Persons who were laid off as a result of the Covid-19 Pandemic and have received their first payment for unemployment benefit from the National Insurance Services (NIS) are asked to provide the following information to the Labour Department in order to continue receiving their fortnightly payments:

· Occupation

· Last day for which you were paid

· Expected date of re-employment, if known: _____ (dd/mm/yy) to (dd/mm/yy)_____

This information is needed in order to complete your Certificate of Continuous Unemployment Form which would be submitted to the NIS on your behalf. The information is required urgently.
